What is lipedema?

A condition of the fat tissue

With lipedema, fat tissue increases on the legs, the hips and sometimes the arms. It is painful, almost always on both sides, and it affects almost only women. It often starts during hormonal changes, in puberty, in a pregnancy or in menopause. Family history plays a role as well.

Being overweight does not cause lipedema. It can make the symptoms worse, though. That is why the clinical guideline names a healthy diet as an important building block, alongside compression, exercise and medical treatment.

For the curious: pain, studies and how it works

Pain: In the best study so far, 70 women with lipedema ate 1,200 kcal a day for eight weeks, half of them low in carbohydrates (Lundanes 2024, randomised). In the low carb group the pain went down considerably more, regardless of how much weight the women had lost. A review of seven studies comes to the same result (Amato 2024). In a Norwegian pilot study the pain decreased on keto and rose again when the women went back to more carbohydrates (Sørlie 2022).

Less water in the tissue: The body stores carbohydrates as glycogen, and every gram of glycogen binds around three to four grams of water. When the stores empty, the body releases that water. This can reduce the feeling of tightness in swollen tissue.

Less inflammation: Signs of inflammation can be found in lipedema tissue. Low blood sugar slows the inflammation switch NF-κB and with it messengers such as prostaglandin E2, which increase pain. The ketone body beta-hydroxybutyrate also inhibits another inflammatory pathway, the NLRP3 inflammasome (Youm 2015).

Good fats: Omega 3 fatty acids from fish, linseed oil and walnuts dock onto receptors that turn inflammatory responses down (PPARs). That is why they are in our recipes.

Low insulin levels: Insulin slows fat breakdown. When the level drops, the body can reach its fat reserves more easily (Keith 2021).

The studies are still small and short. Science is currently working out how these mechanisms interact in lipedema exactly.

Who developed the program?

Does the plan replace lipedema treatment?

No. The plan supports your nutrition. Compression, exercise and medical treatment remain important.

Why ketogenic and dairy free?

The 2024 S2k guideline on lipedema lists ketogenic nutrition as a possible recommendation. We suggest ketogenic and dairy free as a start. You can choose any other carbohydrate level and eat dairy, and we adjust everything to you.

I also have Hashimoto's or insulin resistance. Does that work?

Yes. You tell us everything in the questionnaire. All entries go into the same plan.

What does the questionnaire ask?

After your order you answer the questionnaire in the app, which takes around five minutes. It asks for your goal, height, weight, age and everyday life, your sport, your way of eating (everything, vegetarian, vegan, pescetarian, paleo or AIP) and your carbohydrate level. On top of that, 19 allergies and intolerances, around 700 ingredients you do not like, and conditions such as lipedema, Hashimoto's, insulin resistance or diabetes. Is something that matters to you missing? Then write it into the free text field or send our support a note at info@foodpunk.de. We can usually make it work for you.

Do I have a liposuction coming up?

Write to us beforehand at info@foodpunk.de. In the four weeks before and after the surgery your plan works with moderate carbohydrates and without a calorie deficit. After that it continues as before.

Is there a way to talk to others?

Yes. The lipedema program comes with a private community and a 30 day companion course in the app.

Can I change my entries later?

Yes, in the app. After 28 days a new calculation is free, because the body needs around four weeks to adapt. All recipes are then worked out with your new values. Send special requests to info@foodpunk.de.
